Complete the appropriate reporting form within 48 hours of an accident/incident.

- Non-Student/General Incident Form - (use for faculty, staff, student worker, contractor, or visitor incidents; this includes worker's compensation claims)
- Vehicle Accident Form (including golf carts, boats, trailers)
- Laboratories (including machine shops, art studios)
- Indoor Air Quality and Mold Concerns
Helpful Terminology
Accidents are events that result in physical harm and/or property damage. Examples:
- Injuries
- vehicle accidents
- fire causing damage
Incidents and near- misses are events that could result in physical harm and/or property damage. Examples:
- Chemical spills
- slips/falls
- fire quickly extinguished
Safety concerns are conditions that are unsafe or unhealthy. Examples:
- Uneven walkways
- holes
